Indianapolis Care Coordinator. 

Pay Rate: $18.00

Job Type: Contract to Hire

Responsibilities:

  • Serve as an ambassador for the companies mission, vision and values
  • Assist teams with individual related needs by providing communication
  • Follow up to enhance quality of care management services
  • Complete assessments and care plan development tasks
  • Serve as a client’s advocate

Qualifications:

  • High school diploma REQUIRED
  • Associates degree in human services, psychology, nursing, or related preferred
  • Computer experience; email, Microsoft Office  REQUIRED
  • Healthcare experience preferred
  • 2 years relevant experience in medical, care management setting
